24 Hour Skilled Nursing Facility Services

You or your loved one will have access to 24-hour care by licensed professionals – RN’s, LPN’s, occupational, physical and speech therapists.

A dedicated team will work together to develop a care plan to help you or your loved one achieve and maintain the highest level of ongoing health and wellness. While you’re in our long-term skilled nursing facility, comfort is our main priority – we want you to feel physical comfort, find comfort in our staff’s expertise, and experience the comfort of new friends and our warm environment.

Rehab & Therapy

Work to achieve your goals for rehabilitation with our multidisciplinary team. Each resident receives a customized rehabilitation program to accelerate the healing process and help them find comfort sooner. Rehabilitation programs might include one or a combination of the following therapies:

  • Physical Therapy to improve mobility and balance.
  • Occupational Therapy to improve basic tasks such as dressing and bathing and to simulate tasks unique to the individual, such as feeding a pet.
  • Speech Therapy to improve cognition, reception and expression language, swallowing and eating.

Alzheimer’s and Dementia Care

Stratford welcomes patients diagnosed with Alzheimer’s and related conditions to a simplified, routine lifestyle with 24 hour skilled nursing care. Our fully secured, Memory Unit offers varying levels of independence and offers:

  • Daily activities to stimulate individuals mentally and physically
  • Assistance with bathing, continence, grooming and dressing
  • Three chef-prepared meals per day
  • Medication management
  • Weekly housekeeping
  • Scheduled transportation

Diabetes Care

With a combination of medication, education and lifestyle changes, it is possible to control diabetes and improve the quality of life. All residents receive a customized diabetes care plan including:

  • Educational programs for residents and their loved ones to manage the disease on a day-to-day basis
  • Personalized therapy programs and guided exercise classes to improve mobility and functionality
  • Dietary planning and management to meet caloric and nutrient requirements
  • Medication management

Long-Term Hospice Care Center

After working with your family to select a hospice care team, we can provide end of life care at our facility. Our staff work with your hospice provider to ensure your loved one is comfortable, and has the right level of assistance and education for daily living.

We strive to make the hospice care rooms, located on our assisted living floor, as comfortable as home.

End of life hospice care can be arranged by working with our team here at Stratford.

Cardiac Management

Cardiac Management at Stratford is designed to meet your individual medical and rehabilitation goals. Our skilled nursing staff will help you to stabilize and manage your cardiac condition, while monitoring the following:

  • Medications
  • Blood pressure
  • Cholesterol
  • Weight and physical activity
  • Mood

Stroke Care

Our skilled nurses, therapists and social services work as an interdisciplinary team to help stroke recovery patients meet their individual goals. Inpatient rehabilitation engages patients in a variety of therapies – physical, occupational and speech – to shorten recovery time and help patients return to their highest level of mobility and functionality possible.

Stratford’s skilled nursing and rehabilitation facilities offer:

  • At least 30-45 minutes of rehabilitative therapy five days per week
  • A monthly activities calendar, providing opportunities to practice skills
  • Medication, rest, physical activity and diet management to reduce complications and risk of another stroke

Pulmonary & Respiratory Care

Our comprehensive pulmonary and respiratory care program addresses many conditions that inhibit respiratory functions and primarily helps residents recognize symptoms, identify and control triggers, and ultimately to breathe easier.

The skilled nursing staff will create a customized respiratory care plan, utilizing daily assessments and therapies, including but not limited to:

  • Oxygen therapy and monitoring
  • Pulse oximetry
  • CPap and BiPap care
  • Aerosol treatments

Wound Care

Our comprehensive wound care program offers a variety of services, including but not limited to:

  • 24 hour skilled nursing support for post-surgical, acute care, arterial and venous wounds or burns
  • Negative pressure therapy
